Functional fracture bracing, described in 1967, 1 was inspired by the patellar-tendonbearing prosthesis which had recently been developed for below-knee amputees. This prosthesis had eliminated the traditional thigh corset and allowed the transfer of weight-bearing stresses from the soft tissues of the thigh to the patellar tendon and condyles of the proximal tibia. Anticipating that by a similar mechanism, a below-knee weight-bearing cast, moulded like a patellar-tendon-bearing prosthesis, would prevent shortening in axially unstable tibial fractures, the first below-knee functional cast was constructed. 1 Contrary to traditional practice, this new cast allowed full movement of the knee but immobilised the ankle. However, shortly afterwards it was realised that inclusion of the ankle and foot was unnecessary. The ankle was therefore freed and within a short time pre-fabricated plastic braces became an accepted form of treatment. 2

Functional bracing of humeral shaft fractures. A review of clinical studies.

Functional bracing has been widely accepted as the gold standard for treating humeral shaft fractures conservatively. We conducted a literature review to verify the efficacy of this treatment method. Sixteen case series and two comparative studies fulfilled the criteria set. Analysis of these clinical studies showed that humeral shaft fractures when treated with functional bracing heal in an av...

Own clinical experience with functional bracing for treatment of pseudarthrosis and delayed union of the tibia.

BACKGROUND Functional bracing is a proven method of treatment of tibial fractures. Since 1998 we use individually produced braces made of "combicast" according to Schuren technique. The brace greatly limits the angulation and displacement allowing the axial compression, promoting the physiology of fracture healing. MATERIAL AND METHODS Functional bracing was applied in 51 patients, ageing fro...

No evidence for the effectiveness of bracing in patients with thoracolumbar fractures

BACKGROUND AND PURPOSE The use of braces is widespread in patients with thoracolumbar fractures. The effectiveness of bracing, however, is controversial. We sought evidence for the effect of bracing in patients with traumatic thoracolumbar fractures based on outcome and length of hospital stay (LOS). Furthermore, we evaluated the incidence of complications of bracing. METHODS An electronic se...

A Pre-fabricated Bracing System for the Management of Humeral Shaft Fractures: Experience of a Centre in the United Kingdom.

BACKGROUND The aim of our study is to evaluate the ability of a pre- fabricated humeral bracing system in providing sufficient stability to fracture union and carry out a cost analysis comparing bracing versus surgical fixation for these fractures. MATERIAL AND METHODS A review of humeral shaft fractures treated with a pre-fabricated humeral bracing system was undertaken. RESULTS 20 humeral...

The functional outcome of surgically treated unstable pelvic ring fractures by open reduction, internal fixation

Abstract Background: Unstable Pelvic fracture, a result of high energy antero-posterior compression injury, has been managed based on internal fixation and open reduction. The mode of fixation in Unstable Pelvic fracture has, however, been a subject of controversy and some authors have proposed a need to address the issue of partial breach of the pelvic ring elements in these injuries. This st...
